Heartline Staffing is recruiting a Nurse Practitioner for a full-time primary care opportunity in Peoria, Illinois. This position provides value-based healthcare to local Medicare patient populations and is supported by a multidisciplinary care team so providers can focus on quality of care, preventive medicine, and patient outcomes.

Compensation

  • Base Salary: $120,000–$140,000
  • Incentive Bonus: Up to $8,000
  • Additional compensation flexibility may be available depending on experience, location, and language needs
  • Sign-on flexibility available

Schedule & Patient Volume

  • Monday–Friday
  • 8:00 AM–5:00 PM
  • Approximately 13–16 patients per day
  • Approximately 20-minute regular visits
  • Approximately 40-minute new-patient and full diagnostic visits
  • Patients are generally seen approximately every 3 months, with more frequent visits for complex cases as needed

Position Overview

The Nurse Practitioner will provide primary care within a value-based healthcare model focused on preventive medicine, patient outcomes, and reducing avoidable hospital admissions.

Providers are supported by an established care team that includes a scribe, Medical Assistant, and Social Worker.

Clinical Support & Resources

  • Average of 3–5 providers per clinic
  • Scribe support

  • On-site laboratory services with a phlebotomist
  • Access to RubiconMD for specialist support
  • EMR systems: Canopy and Greenway

Patient Experience

  • Value-based primary care model
  • Transportation available for patients within 5 miles of the clinic
  • Community room activities may include exercise, events, yoga, dance, and spa days

Requirements

Requirements

  • Master of Science in Nursing, Doctor of Nursing Practice, or Nurse Practitioner Certificate Program graduate REQUIRED
  • National Nurse Practitioner certification in an accepted specialty REQUIRED
  • Active, non-probationary Nurse Practitioner license appropriate for practice in Illinois REQUIRED
  • Active DEA license REQUIRED

Accepted Nurse Practitioner Certifications

  • Family Nurse Practitioner
  • Adult-Gerontology Primary Care Nurse Practitioner
  • Adult Nurse Practitioner
  • Gerontological Nurse Practitioner

Preferred

  • Understanding of HCC documentation
  • ICD-10 coding knowledge
  • Experience with Health Risk Assessments (HRAs)
