Job Description
Transition Home Healthcare is an elite home health agency serving Houston, TX and surrounding areas since 2010. We are a group of passionate people dedicated to delivering the highest quality services to our patients, families, team members, and the community. Our team is growing and looking for a dedicated:
Physical Therapist (PT)
Shifts Available: PRN & FT
Service Areas with Openings: Northeast, Northwest, Southwest
Responsibilities
- Facilitate the rehabilitation of patients with emotional, physical or mental disabilities.
- Implement physical therapy care plans.
- Coordinate care with the home healthcare team.
- Educate patients and family members about health conditions.
- Prepare timely, complete, and accurate clinical notes.
- Communicate effectively with patients, caregivers, healthcare providers, office support, and other care team members.

Qualifications
- Physical Therapist in the state of Texas.
- Home health experience highly preferred.
- Knowledge of home health regulations and general healthcare compliance.
- Experience with HCHB EMR. (preferred)
- Able to handle physical workload.
- Good driving record.
- Excellent communication and people skills to effectively interact with patients, families, and healthcare team members.

Licensure & Certifications
Candidates must be a licensed Physical Therapist in the state of Texas with a good driving record. Experience in home health and familiarity with HCHB EMR software are highly preferred.
