We are seeking a Full Time (40 hours per week) provider for our Uptown Rehabilitation Center located in Albuquerque, NM.

The Advanced Practice Provider collaborates with center leadership and medical directors to deliver primary and urgent care services to nursing facility residents. They are responsible for improving patient health outcomes and maintaining high-quality care standards within the facility.
Candidates must hold a valid license as a Nurse Practitioner or Physician Assistant with relevant certifications such as NCCPA or FNP. A minimum of two years of experience working with adult or geriatric populations is preferred.
