The Advanced Practice Provider collaborates with the medical director and center leadership to deliver primary and urgent care services to nursing facility residents. They are responsible for achieving center-specific clinical goals and ensuring high-quality health outcomes for the patient population.
Candidates must hold a Master's degree in Nursing and be certified as a Nurse Practitioner or Physician Assistant with active state licensure and DEA registration. A minimum of two years of experience working with adult or geriatric populations is preferred.
