We are seeking an experienced Nurse Practitioner with exceptional communication and relationship-building skills to provide independent palliative care services, focusing on symptom management, goals-of-care discussions, and patient-centered support in the home setting.
This role serves patients throughout Delaware and Montgomery Counties, allowing you to make a meaningful impact where patients are most comfortable. Previous home care, hospice, or palliative care experience is preferred.

Position: Nurse Practitioner, Palliative Home Care
Shift: Full Time, 40 hours/week, Day Shift. No Holidays. On call rotation weekends.
Experience
- 2 years Nurse Practitioner experience preferred in palliative home care.
- Minimum of 2 years Nursing experience in palliative home care preferred.
Education
- Completion of a Nurse Practitioner program required.
- MSN Required.
Licensures/Certifications
- Licensed by the Pennsylvania State Board of Nursing Examiners as a Certified Registered Nurse Practitioner.
- Provider BLS (American Heart Assoc/American Red Cross) required.
- Require ACLS certification.
- May require PALS (pediatric advanced life support).
Successful completion of the MLH credentialing process through the Medical Staff office, with temporary or permanent privileges approved.
Key Responsibilities
The Nurse Practitioner will provide independent palliative care services, including symptom management and goals-of-care discussions in a home setting. They will also collaborate with physicians and the healthcare team to lead quality and evidence-based care initiatives.
Requirements
Candidates must hold an MSN and be licensed as a Certified Registered Nurse Practitioner in Pennsylvania. A minimum of two years of experience in palliative home care is preferred, along with BLS, ACLS, and potentially PALS certifications.
