Make an Impact! The Hospice and Palliative Social Worker provides psychosocial support and counseling for patients and families facing life limiting illness. The social worker coordinates psychosocial and community resource needs in collaboration with the Hospice and Palliative Care Interdisciplinary Group (IDG).
The hospice and palliative Social Worker demonstrates critical thinking, analysis, and clinical judgment in all aspects of hospice and palliative care delivery for patients and families experiencing life limiting illness.
This includes addressing the social, emotional, financial, and spiritual needs of patients and families to improve quality of life and reduce suffering.

Position: Social Worker, Hospice and Palliative Care
Shift: Full Time Day Shift; 8:30 am to 5:00 pm; Monday through Friday
Experience
One year social work experience in health care setting preferred. Hospice or palliative care experience preferred.
Education
Master's Degree from an accredited School of Social Work.
Licensures/Certifications
1. Current licensed Social Worker in the state of Pennsylvania required.
2. Valid driver's license and automobile insurance.
3. Current CPR certification.
4. Certification from the Academy of Certified Social Workers or Hospice and Palliative Social Work preferred.
Key Responsibilities
The Social Worker provides psychosocial support and counseling to patients and families facing life-limiting illnesses. They coordinate community resources and collaborate with the interdisciplinary group to develop personalized care plans.
Requirements
Candidates must hold a Master's degree from an accredited School of Social Work and possess a current Pennsylvania Social Worker license. One year of healthcare experience is preferred, along with a valid driver's license and CPR certification.
