SUMMARY: The physician provides holistic clinical care to OCHC patients by considering socio-economic, language, and other barriers to care. Working in a team environment, the physician ensures that patients receive comprehensive, wrap-around services and partners with the patient to maximize their health and well-being. OCHC physicians actively participate in the QA/QI program and other OCHC initiatives and programs. The position reports to the Chief Medical Officer.
Essential Functions
- Provides direct clinical care to patients.
- Communicates effectively and respectfully with patients from various cultural, ethnic, economic, language, racial, and other backgrounds.
- Performs minor procedures.
- Develops patient care plans, including diagnostic testing, treatments, consultations, and follow-up.
- Manages patients to ensure continuity of care and follow-up on chronic issues.
- Works to ensure that patients receive whole-person care by making appropriate referrals to OCHC’s mental health and case management, eye, chiropractic, and dentistry teams.
- Considers the patient’s social and structural determinants of health, including historical trauma, structural racism, and biological, social, psychological, and environmental factors in providing patient care and recommending treatments.
- Serves and participates in various QA/QI initiatives and OCHC committees.
- Attends and participates in OCHC provider team meetings.
- Participates in performance improvement (PI) peer review auditing activities.
- Maintains adequate EHR charting and documentation of patient care in a timely fashion.
- Practices within the scope of practice for the specialty and works cooperatively with other specialists and staff.
- Follows the general policies and procedures of Open Cities Health Center, including but not limited to HIPAA policies, pain management policy, code of conduct, and communication policies.
- Provides leadership and/or mentorship to students, trainees, and other clinical staff.
- Serves as a clinical consultant to other areas of the organization and provides clinical guidance for various initiatives as necessary.
- Other duties as reasonably assigned.
- Adheres to the OCHC’s Standards and Expectations of Clinical Providers.
Essential Knowledge, Skills and Abilities
- M.D. or D.O. degree from an accredited medical school.
- Board-certified in Family Practice, Internal Medicine, Pediatrics, or Psychiatry.
- Current State of Minnesota medical license.
- Current DEA License.
- Demonstrated competence and comfort in working with racially, culturally, and economically diverse patients and staff.
- History of positive working relationships with providers, staff, and others
- Strong interpersonal and professional communication skills.
- Ability to meet standard FQHC production targets.
Highly Useful Competencies and Characteristics
- Master’s degree and/or experience in public or population health.
- Experience working in an FQHC or similar clinic.
- Language proficiency in Spanish, Hmong, or Somali.
- Experience working with interpreters.
- Knowledge of and desire to work within a public health/community medicine service delivery model.
- Experience treating substance use disorders and co-occurring mental health-substance use disorders.

Requirements
Candidates must hold an M.D. or D.O. degree and be board-certified in Family Practice, Internal Medicine, Pediatrics, or Psychiatry. A current Minnesota medical license and DEA license are required, along with demonstrated competence in working with diverse patient populations.
