Job Description
A leading home health organization in San Marcos, California is seeking an RN Case Manager to join its growing clinical team!
Pay: $55.00 - $65.00/hour
RN Case Manager Opportunity
- Full-time, permanent, direct-hire position
- Monday-Friday, 8:00 AM - 5:00 PM schedule
- Weekend and after-hours on-call rotations
- $12,000 sign-on bonus available

RN Case Manager Responsibilities
- Complete patient assessments and determine home care needs
- Develop and revise individualized plans of care
- Administer medications and treatments as ordered by physicians
- Educate patients and families regarding treatment plans and healthcare needs
- Coordinate care with physicians and interdisciplinary healthcare teams
RN Case Manager Qualifications
- Current California RN license required
- Minimum of one year of recent acute care experience required
- Current BLS/CPR certification required
- Valid driver's license and auto insurance required
- Home health experience preferred

Key Responsibilities
The RN Case Manager will conduct patient assessments, develop individualized care plans, and coordinate care with interdisciplinary teams. They are also responsible for administering treatments and educating patients and their families on healthcare needs.
Requirements
Candidates must hold a current California RN license and have at least one year of recent acute care experience. A valid driver's license, auto insurance, and current BLS/CPR certification are also required.
