Job Description
A leading home health organization in Portland, Oregon is seeking an RN Case Manager to join its growing clinical team!
Pay: $55.00 - $60.00/hour
RN Case Manager Opportunity
- Full-time, permanent, direct-hire position
- Monday-Friday, 8:00 AM - 5:00 PM schedule with weekend rotations
- Guaranteed weekly pay
- Mileage reimbursement provided

RN Case Manager Responsibilities
- Complete initial patient assessments and develop comprehensive plans of care
- Regularly evaluate patient conditions and revise care plans as needed
- Utilize assessment findings to establish nursing diagnoses and treatment goals
- Administer medications and treatments as prescribed by physicians
- Educate patients and families regarding care plans, treatments, and health management
- Coordinate discharge planning and ensure continuity of care
RN Case Manager Qualifications
- Current Oregon Registered Nurse (RN) license required
- Minimum of one year of recent acute care experience required
- Current BLS/CPR certification required
- Valid driver's license and auto insurance required
- Associate's or Bachelor's Degree in Nursing required
- Home health experience preferred

Key Responsibilities
The RN Case Manager will conduct initial patient assessments, develop comprehensive care plans, and coordinate discharge planning. They are also responsible for administering treatments, educating patients and families, and ensuring continuity of care.
Requirements
Candidates must hold a current Oregon Registered Nurse license and have at least one year of recent acute care experience. An Associate's or Bachelor's degree in Nursing, a valid driver's license, and BLS/CPR certification are also required.
