Key Responsibilities
The Registered Nurse will lead the coordination of patient care across the continuum from pre-admission through post-discharge. They will act as a liaison between the patient, family, and medical staff to ensure appropriate resource utilization and timely discharge planning.
Requirements
Candidates must hold an active California Registered Nurse license, a Case Management certification, and Basic Life Support (BLS) certification. A minimum of 2 years of recent hospital-based case management experience is required.
