Key Responsibilities
The RN Case Manager coordinates and delivers patient care, conducting nursing assessments and developing individualized plans of care. They also facilitate interdisciplinary group conferences and maintain accurate clinical documentation to ensure continuity of care.
Requirements
Candidates must hold a current, active, and unrestricted compact Registered Nurse (RN) license and possess at least 1-2 years of relevant nursing experience. A diploma or associate degree in nursing is required, with a bachelor's degree preferred.
