Key Responsibilities
The RN Case Manager is responsible for providing patient-centered hospice care, including conducting assessments and developing individualized plans of care. They collaborate with the interdisciplinary team to manage symptoms, provide education, and ensure high-quality nursing interventions for patients and families.
Requirements
Candidates must hold an ADN or BSN degree and possess a current, unrestricted Registered Nursing license in the state of employment. At least one year of nursing experience is preferred, along with a valid driver's license and reliable transportation.
