Key Responsibilities
The Nurse Case Manager will provide telephonic case management, facilitate referrals, and assess social determinants of health to support member care. They are responsible for managing individualized care plans and ensuring timely communication with healthcare providers and community resources.
Requirements
Candidates must hold an active Registered Nurse (RN) license with Multi-State Compact Licensure and have at least 2 years of clinical case management experience. Experience working with older adults and proficiency in remote work tools are also required.
