Key Responsibilities
The Case Manager is responsible for the assessment, planning, facilitation, and advocacy of comprehensive health care needs for patients to ensure quality and cost-effective outcomes. They collaborate with interdisciplinary teams to manage complex medical and social needs while facilitating transitions between care settings.
Requirements
Candidates must hold a Bachelor's degree in nursing or a related field, or a Master's degree in Social Work, along with at least three years of clinical experience. A valid, unrestricted State of Michigan license as a Registered Nurse or Licensed Social Worker is required.
