Key Responsibilities
The Case Manager-RN coordinates comprehensive patient care and ensures seamless transitions between different levels of care. They are responsible for developing personalized care plans, facilitating discharge planning, and collaborating with interdisciplinary teams to optimize patient outcomes.
Requirements
Candidates must hold a valid Michigan Registered Nurse license and have at least three years of recent acute care nursing experience. A Bachelor's degree in Nursing is preferred, and certification in Case Management (CCM or ACM) is required within one year of hire.
