Key Responsibilities
The Case Manager is responsible for the assessment, planning, and coordination of care for patients with complex medical and social needs to optimize health outcomes. They facilitate interdisciplinary care conferences and advocate for patients to ensure safe transitions across the care continuum.
Requirements
Candidates must hold a Bachelor's degree in nursing or a related field, or a Master's degree in Social Work, along with at least three years of clinical experience. A valid, unrestricted State of Michigan license as an RN or LMSW is required.
