Key Responsibilities
The RN Case Manager will conduct in-home assessments, develop individualized care plans, and coordinate nursing services for a frail elderly population. They will also collaborate with a multidisciplinary team at the day center clinic and maintain thorough clinical documentation.
Requirements
Candidates must be graduates of an accredited nursing program with an active RN license in Pennsylvania and at least one year of experience with frail elderly populations. A valid driver's license is required for local travel to patient homes.
