Key Responsibilities

Conduct telephonic case management assessments and develop individualized care plans to address members' medical, behavioral, and social needs. Collaborate with interdisciplinary teams to coordinate care, facilitate referrals, and ensure compliance with regulatory requirements.

Requirements

Requires an active, unrestricted RN license and at least 3 years of clinical nursing experience. Candidates must possess an associate degree in nursing and the ability to maintain a secure, high-speed home office environment.
