Key Responsibilities
The Registered Nurse Case Manager oversees patient care plans, coordinates multidisciplinary services, and provides skilled nursing care in home settings. They are responsible for accurate clinical documentation, patient education, and supervising nurse aides and LPNs to ensure quality outcomes.
Requirements
Candidates must hold a current Registered Nurse license and have at least one year of experience in an acute care setting. A valid driver's license and reliable auto insurance are required for travel to patient locations.
