This position offers weekends off. Work with Sanctuary Hospice in Cincinnati, Ohio, United States.
The RN Case Manager will provide hands-on hospice nursing care and coordinate services for patients and their families within the community. They will also collaborate with the Clinical Director to assess patient eligibility and participate in an interdisciplinary care team.
Candidates must hold a current unencumbered Registered Nurse license and have at least one year of experience in a medical-surgical or acute care setting. Proficiency in EMR systems, a valid driver's license, and current CPR certification are also required.
