The RN Case Manager will provide hands-on hospice nursing care and coordinate services for patients within the Naperville community. They will collaborate with the clinical team to assess patient eligibility and develop personalized care plans that ensure comfort and dignity.
Candidates must hold a current, unencumbered Registered Nurse license and have at least one year of experience in a medical-surgical or acute care setting. Proficiency with EMR systems and a valid driver's license with reliable transportation are also required.
