This position offers weekends off. Work with Crescent Hospice in Hartsville, South Carolina.
The RN Case Manager will deliver hands-on hospice nursing care and coordinate services to ensure patient comfort and quality of life. They will also collaborate with the Clinical Director to assess patient eligibility and provide ongoing support and education to patients and their families.
Candidates must hold a current unencumbered Registered Nurse license and have at least one year of experience in a medical-surgical or acute care setting. Proficiency in EMR systems, a valid driver's license, and current CPR certification are also required.
