Telephonic RN Case Manager, SNP (Bilingual Preferred)

Key Responsibilities

The RN Case Manager coordinates care for members with complex and chronic needs by assessing health status and implementing treatment plans. They serve as a primary resource for members and their families while collaborating with interdisciplinary teams to resolve barriers to care.

Requirements

Candidates must hold an active, unrestricted RN license in California and possess at least two years of clinical experience along with one year of case management experience. Proficiency in PC-based software and strong communication skills are required to effectively manage member care and analyze reports.
