JOB TITLE: Home Health RN Supervisor / RN Case Manager & Recertification Nurse (RNCM) This position will be hired as PRN or Part-Time only. Additional Part-Time ihours are also available with a 2-hour travel radius to "Secondary" locations listed below.

The RN Case Manager coordinates and re-evaluates home health care services while providing direct patient care and ensuring clinical documentation compliance. They also supervise a team of nurses and aides to ensure optimal patient outcomes and effective communication between physicians and families.
Candidates must be a graduate of an accredited School of Nursing with an active Ohio RN license and at least one year of home health experience. Proficiency in OASIS assessments, strong leadership skills, and reliable transportation for travel within the service area are required.
