Key Responsibilities
The RN Case Manager oversees the patient's plan of care, coordinates multi-disciplinary services, and provides skilled nursing care in the home. They are responsible for accurate clinical documentation, patient education, and supervising nurse aides and LPNs.
Requirements
Candidates must hold a current Registered Nurse license in the state of employment and have at least one year of acute care experience. A valid driver's license and auto insurance are required for travel to patient homes.
