Key Responsibilities
The RN Case Manager will deliver hands-on hospice nursing care and coordinate services for patients within a designated community territory. They will also collaborate with the interdisciplinary team to assess patient eligibility and provide compassionate support to patients and their families.
Requirements
Candidates must hold a current unencumbered Registered Nurse license and have at least one year of experience in a medical-surgical or acute care setting. Proficiency in computer programs and EMR systems, along with a valid driver's license and reliable transportation, is required.
