Key Responsibilities
The In-Patient RN Hospice Case Manager provides holistic, family-centered end-of-life care and manages patient symptoms within an inpatient setting. They are responsible for clinical documentation, coordinating transitions of care, and collaborating with the interdisciplinary team to support patient and family needs.
Requirements
Candidates must be a Registered Nurse with a minimum of 3 years of inpatient clinical experience, preferably in hospice. A valid driver's license and auto insurance are required, along with the physical capability to perform all job demands.
