Key Responsibilities
The RN Case Manager coordinates and facilitates quality, cost-effective care for ESRD and CKD patients to minimize healthcare fragmentation. They work autonomously and collaboratively with healthcare teams to manage individualized care plans and support patients through complex transitions.
Requirements
Candidates must have an A.D.N. degree or higher, a current RN license, and at least five years of clinical nursing experience. Proficiency in renal nursing, case management, and basic computer skills is required, along with a valid driver's license and home office setup.
