Key Responsibilities
The RN Case Manager performs patient assessments and collaborates with the care team to develop and implement individualized plans of care. They also supervise LPNs and HHAs while promoting patient independence through education and rehabilitative measures.
Requirements
Candidates must have at least one year of clinical experience as a Registered Nurse and hold a current RN license for the assigned state. A valid driver's license, reliable transportation, and current CPR certification are also required.
