Key Responsibilities

The RN Case Manager coordinates and facilitates quality, cost-effective care for ESKD and CKD patients by collaborating with healthcare teams and nephrology partners. This role involves managing individualized care plans and supporting patients through complex healthcare transitions while working both in the field and from a home office.

Requirements

Candidates must hold an A.D.N. degree or higher and a current Registered Nurse license, with at least five years of clinical nursing experience. Proficiency in case management, strong computer skills, and a valid driver's license are required to manage patient care across the assigned territory.
