Nurse Case Manager I

Key Responsibilities

The Nurse Case Manager performs care management for members with complex and chronic needs by assessing, developing, and implementing individualized care plans. They coordinate internal and external resources while monitoring the effectiveness of care to optimize health outcomes across the continuum.

Requirements

Candidates must possess a Bachelor's degree in a health-related field and at least 3 years of clinical experience. A current, unrestricted RN license in the applicable state is required, with multi-state licensure needed if providing services across multiple states.
