Our Community Health Workers, each a licensed nail technician, provide in-home preventive foot care that builds trusted relationships and opens the door to better health above the knee.

Key Responsibilities
Provide routine telehealth visits for Medicare patients and manage personalized care plans for chronic conditions. Collaborate with an integrated care team to ensure thorough documentation and adherence to quality measures.
Requirements
Requires an MD or DO degree with board certification or eligibility in Family Medicine, Internal Medicine, or Geriatrics. Candidates must hold an active medical license in Georgia and be credentialed with major insurance plans.
