The Case Manager, RN conducts nursing assessments and develops personalized care plans for elderly participants within an interdisciplinary team. They are responsible for delivering nursing interventions, coordinating care needs, and educating participants and their families on self-care and disease prevention.
Candidates must hold an unencumbered California RN license and have graduated from an accredited nursing school, with a BSN preferred. At least two years of clinical nursing experience with a frail or elderly population is preferred, along with current BLS and First Aid certifications.
