The RN Case Manager assesses patient needs and coordinates the Plan of Care with the Interdisciplinary Team to provide palliative and supportive care. They perform home visits to monitor patient health, manage symptoms, and educate families on the disease process and care practices.
Candidates must hold a current RN license in the state of practice and have at least one year of hospice or hospital nursing experience. A valid driver's license, reliable transportation, and active BLS certification are required to perform essential home visit duties.
