The Case Manager, RN conducts nursing assessments and develops personalized care plans as part of an interdisciplinary team. They also deliver nursing interventions, coordinate care needs, and educate participants and their families on disease management.
Candidates must be graduates of an accredited nursing school with an unencumbered California RN license. Two years of clinical nursing experience with a frail or elderly population is preferred.
