This position offers weekends off. Work with Hospice of the West in Casa Grande, Arizona.
The Registered Nurse Case Manager provides hands-on hospice care and coordinates services for patients and families within the community. They also collaborate with the clinical team to develop personalized care plans and ensure clinical excellence in end-of-life support.
Candidates must hold a current unencumbered Registered Nurse license and have at least one year of experience in a medical-surgical or acute care setting. Proficiency in EMR systems, a valid driver's license, and current CPR certification are also required.
