RN Case Manager / RNCM (Home Health)

Key Responsibilities

The RN Case Manager conducts initial patient assessments, develops and revises individualized care plans, and provides direct nursing care including medication administration. They also coordinate with families, manage discharge planning, and provide ongoing health education to ensure patient needs are met.

Requirements

Candidates must hold a current and valid state RN license, possess at least one year of recent acute care experience, and maintain BLS/CPR certification. A valid driver's license and auto insurance are required, with a preference for candidates holding an Associate's or Bachelor's degree in Nursing and prior home health experience.
