The Case Manager RN is responsible for assessing, planning, and coordinating care for members with complex medical and social needs to improve health outcomes. They will utilize clinical tools and motivational interviewing to engage members and facilitate wellness through a holistic, interdisciplinary approach.
Candidates must be a Registered Nurse with an unrestricted multi-state/compact license and at least 3 years of clinical practice experience. Additionally, 1 year of medical case management, care coordination, or discharge planning experience is required, along with a dedicated, hardwired home office environment.
