Hospice Team Physician

We are looking for a dedicated physician to join our hospice care team and provide comprehensive medical support to individuals facing serious, advanced, and life-limiting conditions. The Hospice Team Physician plays an important role in helping patients remain comfortable while supporting their personal goals, dignity, and quality of life.

Working as part of an interdisciplinary care team, this physician partners with patients and families as well as nurses, advanced practice providers, attending physicians, and other clinical professionals. The physician provides medical direction for symptom management, medication decisions, care coordination, and ongoing hospice planning.

This role also offers opportunities to consult with community-based providers, contribute to clinical education, and help strengthen the delivery of high-quality hospice services.

Role Details

• Full-time physician opportunity located in Dayton, Ohio
• Responsible for hospice coverage across the Southern Ohio region
• Services may be delivered in patients' residences, nursing or other care facilities, and hospice inpatient locations
• Clinical responsibilities include both face-to-face visits and telehealth
• Participation in weekend and on-call schedules is required
• Physician must satisfy all applicable credentialing requirements within the Southern Ohio service area
• Periodic travel to other locations within Ohio may be necessary

What You’LL Do

• Provide medical evaluation and ongoing management for hospice patients with complex and life-limiting conditions
• Establish, modify, and oversee patient-specific treatment and hospice care plans
• Assess and address pain, symptom burden, and other clinical concerns
• Evaluate medication regimens and make adjustments when medically indicated
• Contribute to interdisciplinary team conferences and coordinated care planning
• Communicate and collaborate with attending providers, nursing professionals, APPs, and other members of the care team
• Offer medical supervision, consultation, and clinical support to advanced practice providers
• Promote complete, accurate, and timely clinical documentation
• Serve as a resource for hospice clinicians and participate in staff education
• Ensure medical records appropriately document terminal diagnoses and continued hospice eligibility
• Participate in clinical quality initiatives, process improvement, and other organizational programs

What You’LL Bring

• Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree
• Unrestricted medical license in the State of Ohio
• Current, unrestricted DEA registration
• Previous experience in hospice, palliative care, hospital medicine, or a comparable clinical setting is preferred
• Background caring for patients with advanced, complex, or terminal conditions is preferred
• Board certification or eligibility in Hospice and Palliative Medicine is preferred
• Current CPR certification
• Excellent clinical decision-making and interpersonal communication abilities
• Proficiency with electronic health records and standard computer applications
• Valid driver's license and acceptable driving record
• Automobile insurance that satisfies applicable requirements
• Ability to travel among patient care locations when necessary
• Availability for assigned weekend and on-call responsibilities
• Ability to successfully complete required employment screening and background verification
