Physical Therapist (PT) Full-Time

NeoPath Health, Inc., a Minnesota based administrator of employer based medical services, is in search of a full-time physical therapist to work at an employer based clinic in the Green Bay, WI area.

Position Summary

The Physical Therapist is an integral part of the NeoPath team. The Physical Therapist works within our clinical team to promote mobility, function, independence, and quality of life for our patients.

They are responsible for evaluating movement and functional limitations, developing individualized plans of care, and providing timely, evidence-based physical therapy services.

Principal Duties and Responsibilities

• Perform comprehensive physical therapy evaluations by reviewing medical history, assessing pain, mobility, strength, balance, gait, posture, range of motion, and functional limitations.
• Develop individualized plans of care with measurable functional goals based on evaluation findings, patient needs, and applicable clinical guidelines.
• Provide therapeutic exercise, neuromuscular re-education, gait and balance training, manual therapy, and other appropriate physical therapy interventions within scope of practice.
• Use therapeutic modalities and other evidence-based techniques, as clinically indicated, to reduce pain and improve function.
• Educate patients and caregivers regarding diagnosis, treatment plans, home exercise programs, injury prevention, body mechanics, and strategies to support safe function and independence.
• Monitor patient response to treatment and modify the plan of care as needed to support progress toward established goals.
• Identify findings that require referral or additional medical evaluation and communicate concerns promptly to the appropriate member of the care team.
• Maintain timely, accurate, and complete documentation of evaluations, treatment sessions, progress, outcomes, and discharge planning in the electronic health record.
• Collaborate with providers and other members of the clinical team to coordinate patient-centered care and support continuity of care.
• Recommend and educate patients regarding appropriate assistive devices, adaptive equipment, and home or workplace modifications when indicated.
• Perform all other duties as assigned.

Qualifications

• Doctor of Physical Therapy (DPT) degree or other physical therapy degree meeting applicable licensure requirements.
• Graduate of a physical therapist education program accredited by the Commission on Accreditation in Physical Therapy Education (CAPTE), or otherwise meeting applicable state licensure requirements.
• Current licensure as a Physical Therapist in good standing.
• Three or more years of clinical physical therapy experience preferred.
• Strong organizational skills and the ability to manage multiple priorities
• Must possess computer skills - Excel, Word, Outlook Occupational recordkeeping system and EHR experience preferred.

Working approximately 40 hours per week No weekends, holidays, or after-hours call.
