Key Responsibilities

The Care Navigator assists patients in navigating the healthcare system by managing referrals, closing care gaps, and coordinating care plans. They act as a primary point of contact and advocate for patients while collaborating with the clinical team to ensure effective self-care management.

Requirements

Candidates must be graduates of an accredited nursing program and hold current RN and BLS certifications. Strong interpersonal, organizational, and communication skills are required, along with basic computer proficiency.
