Key Responsibilities

Serve as the primary point of contact for heart failure patients, facilitating their care plans and providing education on self-management. Conduct patient assessments, monitor progress, and maintain communication between patients, families, and the multidisciplinary care team.

Requirements

Requires a current Oregon and California RN license, with BLS and ACLS certifications. A minimum of 2 years of experience in cardiology, intensive care, or heart failure is preferred.
