Job Description

  • Hospice Associate Medical Director
  • Hackensack Meridian Health
  • Wall, New Jersey

Hackensack Meridian Health (HMH )is seeking a compassionate and collaborative Associate Medical Director of Hospice to join our dedicated interdisciplinary team. In this pivotal role, you will be a champion for quality care, ensuring our patients and their families are supported through their most vulnerable moments. If you are a medical leader who believes in a holistic, team-based approach to symptom management and patient support, we want you on our team.

About the Role

As the Associate Medical Director, you will partner closely with our Medical Director, attending physicians, and our interdisciplinary group (IDG). You will serve as a vital medical resource—guiding complex pain and symptom management, ensuring seamless continuity of care, and shaping the clinical standards of our hospice program.

Responsibilities

Direct Patient Care: Step in as the primary or designee physician when an attending physician is unavailable or wishes to relinquish care, and ensure 24/7 availability to meet urgent general medical needs not covered by the attending physician.

Clinical Leadership: Consult with attending physicians on advanced pain and symptom management, review patient medical eligibility, establish care plans, and provide written certifications and clinical narratives for terminal illness (including face-to-face encounters).

Program Strategy: Drive critical clinical outcomes, including reducing hospital readmissions, optimizing hospice utilization, and ensuring appropriate risk stratification.

Relationship Building: Serve as our "Hospice Champion" and community liaison by partnering with community physicians, specialists, health plans, and potential donors to ensure a cohesive approach to patient care and referral management. You will also provide empathetic communication and counseling to patients and families, helping to gracefully navigate and resolve clinical, ethical, and interpersonal concerns.

Education & Academics: Act as the premier medical resource for our interdisciplinary group during team meetings and case reviews. You will train hospice staff on end-of-life care, drive continuous performance improvement, stay on the cutting edge of hospice research, and leverage the opportunity to teach at our School of Medicine to train the next generation of physicians.

On-Call: Participate in a shared, rotational after-hours on-call schedule to ensure continuous, high-quality care and support for our hospice patients.

Qualifications

  • Doctorate Degree in Medicine (MD) or Osteopathy (DO).
  • Minimum of two (2) years of experience beyond board certification.
  • At least three (3) years of general healthcare experience.
  • A current state medical license in good standing with the medical board.
