Wound Care Nurse Practitioner functions as an independent practitioner pursuant to Section 464.0123, F.S., practicing without written collaborative agreement or physician supervision.

Highly skilled advanced practice nurse specializing in assessment, management, and prevention of complex wounds, ostomies, and continence-related conditions. This role provides expertise in delivering evidence-based care, promoting optimal patient outcomes, and supporting interdisciplinary collaboration.

This Autonomous APRN serves as a clinical leader, educator, and resources for patients, families, and healthcare staff.

  • Duties and Responsibilities may include, but are not limited to:
  • Serves as the designated Provider in the specialized role of wound care expert in assessing, treating and managing diverse wounds to provide healing and improve patient outcomes.
  • Provides clinician oversight of Wound Care Department care delivery throughout CenterWell in the Villages.

Performs initial comprehensive wound assessments, including identification of wound etiology, diagnosis, management and treatment of complex acute and chronic wounds evaluating wound depths, tunneling, vascular status and underlying comorbidities, developing plan of care including advanced dressing recommendations and treatment modalities for Wound Care Manager to implement scheduling and care delivery throughout wound care team.

Serve as a wound care expert to the medical team, referring patients to vascular or plastic surgery, and podiatry when necessary.

In collaboration with Wound Care Manager, leads pressure injury prevention initiatives, including risk assessment, staff guidance, and protocol implementation.

Conducts bedside procedures such as sharp debridement and biopsies, incision and drainage, and skin substitute applications.

Orders targeted pharmacological treatments (such as topical antimicrobials and antibiotics) and diagnostic tests (arterial Dopplers, cultures, and labs).

In collaboration with Wound Care Manager, direct and provide ostomy care services, including postoperative management.

In collaboration with Wound Care Manager, participate in multidisciplinary case reviews to ensure compliance, effective standards of care, continuity and quality of care.

  • Serve as a clinical resource for staff regarding best practices in wound care, ostomy, and continence care.
  • Instruct patient/family regarding medications and treatments.
  • Educates patients regarding health promotion/illness prevention.
  • Recommends appropriate community resources to meet patient/family needs.

Communicates appropriate case management information to other professionals and community agencies. Prepares documentation for medical records including updating patient medical chart by posting examination and test results, diagnosis, medications, and treatment in written/computerized manner.

Participates in peer reviews, chart reviews, staff education, clinical guideline development, and other continuing education and quality assurance activities to demonstrate compliance with standards, regulations, policies, and procedures.

Acts as consultant/collaborator to clinical team on area of specialty.

Compliant with patient confidentiality requirements. Promotes patient advocacy.

1. Active, clear Florida APRN license with official Autonomous APRN designation, or ability to gain designation upon hire having met the requirements of Section 464.0123(1), including: a. Completed a minimum of 3,000 clinical hours under physician supervision within the last 5 years. b.

Completed three graduate-level semester hours (or the equivalent) in differential diagnosis, and three graduate-level semester hours (or the equivalent) in pharmacology, within the last 5 years.

  • Board Certification in Family or Adult.
  • Wound Care certification required or obtained within 6 months of date of hire.
  • Minimum of 3–5 years of clinical experience in wound and/and or ostomy care.
  • Must possess a current CPR/BLS or ACLS certification.

Current health records with the appropriate immunizations or proof of immunity to work in the health care field (hepatitis B and tuberculosis).

Knowledge of advanced practice of nursing include anatomy and physiology, pathophysiology, pharmacology and therapeutics, basic nutrition, behavioral factors, psychosocial/family systems, diagnostic testing, interpretation of results and clinical decision making, alternative treatment modalities.

Knowledge of professional nursing theory, practices and regulations related to evaluating and providing patient care, and professional ethics related to the delivery of nursing care.

  • Knowledge of medical equipment and instruments used to administer patient care.
  • Knowledge of common safety hazards and precautions to establish/maintain a safe work environment.
  • Knowledge of health promotion, health risk identification, and patient education principles and techniques.
  • Knowledge of community, professional, and educational resources.

Skill in oral and written communication including interviewing to gather medical histories; providing presentations, consultations, facilitation, and documentation, e. g., writing medical orders, patient education materials, and medical record updates. May require skill in using computerized health information management systems.

Skill in case management, time management, problem solving, crisis intervention, multitasking, and other organizational matters.

Skill in modifying methods and techniques of professional nursing to provide appropriate nursing care and in initiating appropriate emergency response and investigatory procedures.

  • Ability to establish/maintain effective working relationships with patients, clinical staff, and the public.

Supervisory Responsibilities

  • Clinician provides Wound Care supervision of Wound Care Manager and all Wound Care department staff.
  • Must be technologically skilled and capable of performing virtual supervision activities as needed.

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.

$123,800 - $170,300 per year

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
