As a Hospitalist Nurse Practitioner, you will provide high-quality, patient-centered care to adults throughout their hospital stay. Working within an interdisciplinary hospital medicine team, you will help manage acute and chronic conditions, coordinate treatment, and support safe transitions from admission through discharge.
You will collaborate closely with hospitalist physicians, nurses, specialists, pharmacists, case managers, social workers, and other clinical partners to deliver timely, coordinated care in accordance with state scope-of-practice requirements, hospital policies, and granted clinical privileges.
In this role, your clinical judgment, communication, and commitment to quality will help improve patient outcomes and the hospital experience.
Your Role & Responsibilities
Perform comprehensive histories and physical examinations, formulate differential diagnoses, and develop evidence-based plans of care for hospitalized adults.
Manage acute and chronic medical conditions within the Nurse Practitioner’s scope of practice and granted hospital privileges.
Order, review, and interpret laboratory studies, imaging, and other diagnostic tests; adjust treatment plans based on clinical findings and patient response.
- Conduct daily rounds, reassess patient status, and communicate changes in condition promptly to the care team.
- Prescribe medications and therapeutic interventions in accordance with applicable laws, collaborative or supervisory requirements, and hospital policy.
- Complete admission, progress, consultation, transfer, and discharge documentation accurately and on time.
Coordinate care with physicians, specialists, nursing, pharmacy, case management, social work, rehabilitation, and other disciplines.
Communicate diagnoses, treatment plans, progress, and discharge instructions clearly to patients and their designated support persons.
Develop and support safe discharge plans, including medication reconciliation, follow-up care, and handoffs to post-acute or outpatient providers.
Recognize changes in patient condition, initiate appropriate interventions, and escalate care when needed.
Participate in scheduled shifts, weekend or holiday coverage, and call responsibilities as required by the assigned hospitalist program.
Support quality improvement, patient safety, utilization management, regulatory compliance, and hospital medicine initiatives.
- ED admission screening and avoidance.
- Observation versus inpatient status optimization.
- Return-to-clinic decision making.
- Daily multidisciplinary rounds with discharge barrier management.
- Ensuring follow-up appointments prior to discharge.
- Hospital throughput, capacity management, and utilization review responsibilities.
Use your skills to make an impact
Required
- Master of Science in Nursing or Doctor of Nursing Practice from an accredited Nurse Practitioner program.
- Current, unrestricted Registered Nurse and Advanced Practice Registered Nurse or Nurse Practitioner licenses in the state of practice.
- Current national board certification as a Nurse Practitioner from a recognized certifying body.
Certification and population focus appropriate to the hospital’s patient population and the clinical privileges requested.
Current Basic Life Support and Advanced Cardiovascular Life Support certifications, or ability to obtain them before the start date.
Current prescribing authority and active DEA registration, or ability to obtain them, when required for the role.
- Eligibility for hospital credentialing and clinical privileges.
- Strong clinical assessment, critical-thinking, prioritization, communication, and interdisciplinary collaboration skills.
- Ability to work effectively in a fast-paced hospital environment and use an electronic medical record system.
Preferred
At least one year of Nurse Practitioner experience in hospital medicine, internal medicine, acute care, critical care, or a related inpatient specialty.
Acute Care Nurse Practitioner certification for roles primarily managing acutely ill adult or older adult patients.
Experience with admissions, daily rounding, discharge planning, medication reconciliation, and transitions of care.
- Experience working with medically complex adults and collaborating with multidisciplinary hospital teams.
- Familiarity with inpatient quality measures, documentation standards, utilization management, and patient safety practices.

Pay Range
The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
$123,800 - $170,300 per year
This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
