Key Responsibilities
The Social Worker performs psychosocial assessments for patients and their families to develop outcomes for alleviating symptoms. They provide supportive counseling, psychoeducation, and casework interventions while consulting with the interdisciplinary group.
Requirements
Candidates must be a Licensed Master Social Worker (LMSW) capable of working within a core interdisciplinary team. Professionalism, collaboration, and a commitment to hospice care are essential.
