Job Description
University Home Care Inc is seeking a dedicated and experienced RN Case Manager skilled in home health to join our team in Livonia, MI. This role involves overseeing patient care plans, coordinating with healthcare providers, and ensuring patients receive high-quality home health services tailored to their individual needs.
Responsibilities
- Conduct comprehensive patient assessments and develop individualized care plans.
- Coordinate patient care with physicians, therapists, and other healthcare providers.
- Monitor patient progress and adjust care plans as necessary.
- Educate patients and families on managing health conditions at home.
- Ensure compliance with company policies and regulatory requirements.
- Maintain accurate and timely documentation of patient information.
Requirements
- Registered Nurse (RN) license.
- Proven experience as a Case Manager in skilled home health care.
- Strong knowledge of home health regulations and best practices.
- Excellent communication and organizational skills.
- Ability to work independently and as part of a healthcare team.
- Valid driver’s license and reliable transportation.
