RN case manager hospice must have experience

University Home Care Inc is seeking an experienced RN Case Manager Hospice to join our dedicated team in Livonia, MI. This role is critical in providing quality hospice care and support to our patients and their families, ensuring compassionate and comprehensive case management throughout the care continuum.

Responsibilities

• Provide skilled nursing assessments, care planning, and case management for hospice patients in accordance with company policies and regulatory requirements.
• Coordinate with interdisciplinary team members, patients, and families to deliver individualized care plans.
• Ensure timely and accurate documentation of patient care and case management activities.
• Advocate for patients’ needs and preferences while promoting comfort and dignity.
• Maintain compliance with all regulatory standards and company protocols.
• Communicate effectively with patients, families, and healthcare team members to promote continuity and quality of care.

Requirements

• Valid RN license with current registration.
• Proven experience as a Case Manager in hospice care.
• Strong knowledge of hospice guidelines, regulations, and best practices.
• Excellent communication, organizational, and interpersonal skills.
• Ability to work independently and as part of a multidisciplinary team.
• Compassionate approach to patient care with strong ethical standards.
