- Department/Specialty: Hematology/Oncology
- Schedule: Days | Full time | Monday-Friday 8am-4:30pm
- How you'll make an impact in this role
- Serve as a patient navigator throughout the continuum of care.
- Function as a liaison for management staff, administration, physicians, managed care companies, community organizations, and other customers.
- Coordinate efforts for treatment and promote quality improvement and educational efforts.
- Compile data, tracks outcomes, and make recommendations for process improvement.
- Provide referrals/support for follow-up care for patients who are uninsured or underinsured.
- What minimum qualifications you'll need
Licensure / Certification / Registration
Registered Nurse credentialed from the Indiana Board of Nursing obtained prior to hire date or job transfer date required.
BLS Provider preferred. American Heart Association or American Red Cross accepted.
Education
- Diploma from an accredited school/college of nursing OR required professional licensure at time of hire.

- Oncology Certified Nurse (OCN) preferred
- Two years of experience in Oncology and or Case Management Preferred

Key Responsibilities
The Patient Navigator serves as a liaison between patients, medical staff, and community organizations to coordinate treatment and support services. They are responsible for tracking patient outcomes, managing referrals for underinsured patients, and recommending process improvements.
Requirements
Candidates must hold a valid Registered Nurse credential from the Indiana Board of Nursing and possess at least two years of experience in oncology or case management. A diploma from an accredited nursing school is required, while OCN certification and BLS provider status are preferred.
