Assesses, plans, and coordinates multidisciplinary inpatient care to optimize clinical outcomes, manage length of stay, and prevent avoidable readmissions.
Conducts utilization reviews and communicates with third-party payors to secure authorizations and ensure appropriate level-of-care status.
Evaluates post-acute care needs and arranges durable medical equipment, home health, skilled nursing placements, or community resources.
Serves as the primary patient and family advocate, facilitating discharge planning, goals-of-care discussions, and smooth care transitions.
What minimum requirements you’ll need
Licensure / Certification / Registration
Licensed Registered Nurse credentialed from the Texas Board of Nursing required.
BLS Provider preferred. American Heart Association or American Red Cross accepted.
Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.

- Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
